What is navify® My Care?

The navify® My Care app enables you to securely access and view your health data for the Care program in which you were enrolled by your healthcare organization. You can use the navify® My Care app to:

• View the health data captured from your devices, such as a Blood Pressure monitor or a Weight Scale
• Get notifications for the daily activities according to your Care program
• Respond to surveys assigned to you by your healthcare provider
